Submission of user request form is required for any person who needs new access within ClientTrack HMIS.

Before submitting the form, please note the following:

• Limited Course Access – submitting the form will automatically add the new user to the required HMIS New User course in Eduflow. Users will see limited information in the course until the request is further processed by our training team
• Transferring Organizations – users who were recently active in ClientTrack with another organization and who are entering data for the same program type do not need to complete New User Training again
• Project Type – this should match the HMIS project type(s) of the programs the user will enter data for, including Coordinated Entry (CAS)
