Question: What can I use to document homelessness when DOPSing a client?

Answer: You have three options –

  • A bed stay report (from HMIS);
  • A referral letter (from an outreach worker, a police officer, a pastor, a case manager or the like), attesting that the signatory has seen the client experiencing homelessness;
  • A self-report affidavit. Only one such affidavit may be submitted, and it cannot cover more than 90 days.

Remember, you can use one, two or all three types of documents, however, at least one must be current.
